A completed Fit Form is required before each new appointment.

Completing this form helps us prepare for your appointment, confirm you are coming, and to assure sanitation and mitigation period of everything you may try on.  The form is sent to you automatically 48hrs before your scheduled appointment and needed back at least 12hours before your appointment. Two reminders are sent and if we do not receive the confirmation Fit Form, your appointment is automatically canceled.
